Canopy Cancer Collective

Canopy Cancer Collective is a nonprofit that works with leading healthcare systems to promote comprehensive, coordinated care across the entire pancreatic cancer care journey, preventing patients and their families from getting lost in the system.  

Supported by the infrastructure and resources of the Canopy Cancer Collective, participating cancer centers collaborate together to share data and best practices to accelerate improvements in care delivery and outcomes in pancreatic cancer. Canopy also supports care team wellness by working to help ensure that the entire team - doctors, nurses, advanced practitioners, supportive care and wellness specialists, are operating at the top of their license and skill.

Community Health Leadership Council San Joaquin (CHLC)

Established in 2000, the Community Health Leadership Council San Joaquin (CHLC) is a collaborative of key healthcare, education, and business leaders who serve to improve the health and quality of life in the Northern Central Valley. The CHLC examines and assesses the landscape to identify and address critical healthcare needs like improved access to quality healthcare, growing a strong healthcare workforce, and shaping policy by fostering community partnerships and collaboration.

Through topic-specific annual community health forum convenings and ongoing community conversations, the council brings together stakeholders, regional partners, and the community to identify and implement initiatives to reduce negative impacts on individuals' health and their communities.

Healing Our Healthcare Heroes

In collaboration with participating healthcare systems, 1440 Foundation and 1440 Multiversity offers members of healthcare teams a 3 day/2 night program focused on self-care and wellbeing.  Led by certified professionals, the program offers tools for resilience, moral injury, grief and finding joy.  Surrounded by beautiful redwoods and attentive staff, healthcare professionals participate in healthy meals, movement and meditation-based programs, all designed to rejuvenate and revive the spirit.  You can learn more about this program and details for applying here.

Hospice Giving Foundation

Hospice Giving Foundation believes in living fully and dying peacefully. For over 25 years, we have been dedicated to ensuring compassionate, dignified end-of-life care by funding local agencies and programs that promote personal choice, preparedness, and support for individuals and their loved ones. As a leader in a social health movement, Hospice Giving Foundation advocates for cultural attitudes of hope, understanding, and acceptance around end-of-life care. We champion an integrated system of quality resources, evolving best practices, and personalized care to meet the needs of our community. Through grants, education, and advocacy, we help people navigate this critical stage of life with compassion, dignity, and peace.
